This section of the web site is devoted to the forms you'll need to have for the Pikeville Regional Olympiad.

2009-2010 KSO Membership Form

This form is necessary to register one or more teams for the 2009-2010 Olympiad. The form should be returned, along with payment, to the state director at WKU. If you do register for the Pikeville Regional Olympiad please also send email to the regional directors so they are aware of your participation and can include you on upcoming announcements and mailings.

Student Registration Form

This form is necessary to have the names of each team member on file with the regional directors. It is provided as an EXCEL document and should be downloaded as is, filled out typed and returned via email by Friday, February 12, 2010.

Vandalism and Behavior Form

This form is necessary to ensure that each team (students and coach) conduct themselves with an appropriate level of sportsmanship. The form must be signed by each member of the team, that team's designated coach, and the principal of the team's school. The form is to be submitted during check in on the date of the regional Olympiad.

Arbitration Form

This form is used in the event that a coach wishes to make an appeal regarding event supervision or scoring in connection with the Regional Olympiad. The form may be downloaded and brought to the competition. In addition, copies of the form will be available on the day of the competition.
